Man falls to his death while working at Mentor bridge

Upon arrival, first responders reportedly found the man -- identified as Raul Perez -- 'in cardiac arrest with major head injuries.'

MENTOR, Ohio — A 53-year-old man has died after falling approximately 20 feet from an extension lift while it was being lowered amid work at the Hendricks Road bridge in Mentor last week.

A press release from Mentor city officials Tuesday morning says the employee of Beaver Excavating was painting the bridge at the time of the incident, which happened May 11 at 10:56 p.m.

“It appears that the door to the occupant area opened unexpectedly, and he was not wearing a safety harness at the time,” according to a press release from the city of Mentor.

Upon arrival, first responders reportedly found the man -- identified as Raul Perez -- “in cardiac arrest with major head injuries.” He was given care at the scene before being transported to a hospital.

Perez was later pronounced dead.

“OSHA was contacted and responded to the scene to begin an investigation,” according to the press release from the city of Mentor.
