Officials recommend no action against operator of fatal Fireball ride at Ohio State Fair

The Ohio Department of Agriculture has completed its investigation into the fatal accident at the Ohio State Fair in July and recommends that no action be taken against the operator of the Fireball ride.

The Ohio Department of Agriculture has completed its investigation into the fatal accident at the Ohio State Fair in July and recommends that no action be taken against the operator of the Fireball ride.

18-year-old Tyler Jerrell was killed and seven others injured when they were sent flying into the air after an arm on the Fireball ride detached from its base on July 26.

According to the report released Friday, Amusements of America, the operator of Fireball, notified the Department of Agriculture about the incident immediately. However, Amusements of America failed to provide an accident report to the ODA within 24 hours.

Regardless, the ODA's Amusement Ride Safety Division opted not to recommend a fine or revocation of Amusement of America's license for the paperwork violation.

In August, the ride's manufacturer, KMG, determined that "excessive corrosion on the interior of the gondola support beam dangerously reduced the beam's wall thickness over the years."

Prosecutors in Franklin County determined in August that no criminal charges will be filed in this case.
